Membership Information

Your membership cost of $159 (not including any additional applicable taxes) will be due today and then due on the same date of each month hereafter until your membership expires or is terminated in accordance with this agreement. 

We require that you keep a credit card on file for membership payments. 

 

An initial 3 (three) month membership is required. You may only cancel your membership during the initial 3 month period only in the event that a physician certifies that you are unable to receive the membership services. All cancellation requests during the initial term MUST be accompanied by submission of a doctor’s note. Upon approval of your written request for membership cancellation during the initial 3 months, you will be relieved from making any future membership dues payments, but no refunds of membership dues are made. All cancellations are effective 30 days after the date of the final EFT payment has been processed.

 

Payments due prior to the effective date will be charged as scheduled.

Your membership will automatically continue on a month-to-month basis ("renewal term") after the initial 3 month period. You may continue to redeem your treatments as long as your membership is in effect and is paid current. Upon termination or cancellation of your membership, all unredeemed membership services will expire 30 days after the final EFT payment has been processed. Membership benefits do not roll over month-to-month; if benefits are not redeemed, they may not be carried over to the next month.

 

If your credit card on file is declined for any reason, you will be sent an invoice for the TOTAL remaining amount of your membership, which will be due upon receipt. Your account may be sent to a collection agency if the balance is not paid upon receipt. 

 

Exclusions

Membership benefit of 15% off any other aesthetic services applies to skin lasers, microneedling, dermaplaning, body contouring, and vaginal rejuvenation treatments. It does NOT apply to hormone replacement therapy or weight loss programs. 

 

Membership benefit of 10% off products purchased in-office includes nutraceuticals, intimate lubricants, and skin and body care products. It does NOT apply to medications, hormones, or injections.
